Basic items increase your roll modifiers without providing additional bonuses.
In WoW terminology, they'd be considered '
green items'.

Each Basic item gives an additional +1 to Constitution Rating.

HEROIC ITEMS

Eye of the Basilisk

Use this item when an enemy attack that would injure you misses. The attacked instead chooses another hostile target and rolls to have that enemy attack them.

Use this power when an attack would make you stunned, dazed or dominated. You are unaffected by that condition and instead take a -5 penalty to all roll results during its intended duration.

Eye of Awareness

Once Per Event: Gain +5 to initial roll results during the first player phase of an event. These can be used for yourself, or an ally within melee range. The +5 can apply to attack rolls, healing rolls or avoiding damage dealt.

Gain a +5 to rolls involving seeing through preemptive Stealth and attempted ambush attacks.

Goggles of Aura Sight

Once Per Encounter: Choose a target within reasonable range of you, given equipment or ability. Learn the target's current and maximum HP and any attacks the creature can deal.

Eye of the Phoenix

Once Per Event: Roll 3d4. The sum + your Intellect is performed as AoE damage, divided amongst all enemies on the playing field. Strikes a minimum of 2 targets.

Gem of Colloquy

Once Per Event: Read and speak one additional language to communicate with enemies or potential allies, and gain a temporary +2 Wisdom bonus to understand what is being spoken.

Goggles of the Bone Collector

Gain a +5 bonus to your final roll result against an enemy type on which you've landed a killing blow. This effect does not stack, but it does not reset between combat phases.

Goggles of the Hawk

You can make Perception checks to examine targets within your line of sight with no penalty for distance. Deal an additional +2 damage to any target you’ve Perceived. Performing a Perceive on a target takes a full round of action. When a target that has been Perceived is removed from the field within the same combat phase, you may Perceive another target without the full action penalty.

Grimlock Helm

Once Per Event : When activated, you temporarily blind yourself, lowering your Dexterity by 1, but increasing your Wisdom by 2. This effect lasts for two player phases.

Helm of Opportunity

Gain a +5 to attack roll results at the beginning of each combat phase, but only when players have Advantage.

Helm of the Flamewarped

Once Per Event: Use this power when you can make an attack. You are instead dazed until the end of your next turn. If your attack roll hits, the power deals 10 damage with no restriction in lieu of your normal attack. This power cannot be divided among enemies.

Inquisitor's Helm

Once Per Event: Make a full action roll with a DC of 14 using Intellect or Wisdom; on a successful result, pry the answer to one question from the target's mind. If the target doesn't know the answer to the question, you get no answer and the power is spent. Does not work on bosses. DM Discretion.

Stone of Loyalty

As long as you are within reasonable melee range of an ally, they are immune to fear effects and morale penalties, and cannot be made to attack you in specific as a result of an enemy effect.

Quickening Diadem

When you are stunned or dazed, you may perform one attack or one healing roll for half the roll’s result on the next player phase, regardless of the stun/daze's intended duration. (DM Discretion)

Skull Mask

Once Per Event: Increase your Strength or Intellect by the amount of your Wisdom modifier until the end of your next turn.

Stag Helm

Gain a +5 to passive Perception rolls.
Per Event: Attack enemies for 1d4 in addition to your attack, even if the initial attack misses. Attacking enemies with the Stag Helm breaks rotten horn fragments off inside the target, poisoning them for 1d3 damage for 1d3 rounds.

Bear Headdress

Once Per Event: You gain the ability to shapeshift into a bear and temporarily evoke the powers of a Druid of the Claw. You may either gain +5 Strength, but become an unstoppable force of nature, and all damage dealt is also displaced among melee ranged allies; or, gain +3 Wisdom, but fall asleep for two rounds following any healing performed with this acquired modifier. The bear form lasts for two player phases if the player does not fall asleep by healing.

When dazed or stunned, endure the duration of the effect, but heal yourself or another target immediately for +2HP after it ends in addition to an action.

MYTHIC ITEMS

Blasting Circlet

Once Per Event: Make a ranged attack; 2d4 + Dexterity; on a successful hit above 10, the target is knocked backwards out of their current position, interrupting spell channeling.

Circlet of Rapid Casting

Increase the potency done of a concurrent cast in a specific school of magic by +1 for each successful roll of that type performed by members in the party. [ mages receive a bonus if they all cast fire spells, holy priests and paladins working together with the Light, etc. ] This bonus stacks indefinitely, but only for the user of the Circlet, and resets at the end of each combat phase.

Circlet of Second Chances

Once Per Event: Use this power to reroll an attack attempt for Advantage. Despite being used freely once per event, its power will recharge upon rolling a natural 1 to attack or heal.

Clockwork Cowl

Gain a +3 to all Perception rolls.
Once Per Event: Attack the same target twice in one turn, with two separate attack rolls. Each attack roll has Advantage.

Crown of Nature's Rebellion

Once Per Event: Use this power when struck by a melee attack. The attacker takes an amount of damage equal to the damage you just took, and is rooted in place; as a free action, you may distance yourself from the attacker while they are rooted. Roots break free when damaged, or after one enemy phase.

Crown of the World Tree

Once Per Event: When performing a spellcast, roll for Advantage without including modifiers or item bonuses. Add your Intelligence and any item bonuses twice to that result.

Factotum Helm

Once Per Encounter: Temporarily gain +2 in any of the base modifiers (excluding Constitution) until the encounter is over. Choices in modifier affected cannot be changed once made.

Hat of Disguise

You gain the appearance of any humanoid race of the same size as you. Your clothing and equipment alter appearance to reflect this change. The illusion does not alter sound, so a creature listening to you might dispel the illusion.

Once Per Combat Phase: Roll 1d4+1 + your Intelligence or Dexterity, instead of a base attack roll.

Helm of Swift Punishment

Once Per Event: Target an enemy that has dealt damage to you. Immediately retaliate with 2d3 damage to them.

Diadem of Regeneration

Heal for an additional +1HP from each successful healing spell cast upon you.

Mythic items are even more powerful versions of Heroic items.
These could be considered '
purple items'.

Mask of Terror

Once Per Event: Make an attack roll; 1d5 + Strength or Intelligence; on a hit of 10 or greater, the target takes a -2 penalty to its attack rolls until the end of your next turn.

Trickster's Mask

When you make a Stealth roll or an attempted theft roll, roll Advantage + Dexterity.

Headband of Insight

Gain +1 to all roll results against a creature that has had any player’s successful Perception check made against it. Perception checks take up the action of one full Player Phase.

Arcanist's Glasses

Once Per Event: Activate to gain a +4 to Intelligence following a successful spellcast. This effect expires at the end of your next turn.

Crown of Equilibrium

Once Per Event: Heal yourself for the same amount of damage dealt to you the previous Enemy Phase; deal half that damage to an enemy target in addition to your attack roll result.

Crown of Victory

Once Per Event: Immediately heal yourself for double the amount of damage you’ve been dealt in the last enemy phase; if overhealed, designate allies around you to receive healing.

Cyclops Helm

Gain a +5 bonus to Perception checks. Per Event: Spot an enemy that is Stealthed or otherwise hidden without needing to roll for it. (DM Discretion)

Essence of the Wisp

Once Per Event: You may take damage that would K.O. another player, in return for immediately K.O.ing yourself, even if your health is above the knock-out blow. Resurrect yourself at the beginning of the next player phase with 1HP remaining.

Lenses of the Luminary

Gain a +5 bonus to Perception checks. Per Event: Learn the weakness and HP of any enemy that you target with a roll result of 8 or greater, including the Perception bonus granted by this item’s equip. Performing a Perceive on a target takes a full round of action.
